The soccer referee game card below is a PowerPoint file (also available as a PDF).  When you print the file, use a printer that can print greyscale so that the symbols print correctly (if not, the hands on the clocks are too dark and it’s hard to write on top of them).  If you photocopy or laserprint the final versions, the printing will be waterproof.  There are two versions, NCAA and everything else.

To use the card, note the team that kicks off by filling in the circle, and the direction of the kickoff (N, S, E, W) in the little square box.  Write the jersey number in the jersey outline, and the time of the game over the clock symbol.  For misconduct, circle “USB” or “Diss” for Unsporting Behavior or Dissent—any other misconduct, write the reason in the space provided. 

Referees are free to use and make copies of this card for non-commercial uses.  You might want to modify the text on the right side to be appropriate to leagues in your area.  If you have any suggestions for improvements, please email me at pfletcher@jps.net.
